
Sports Physical Information

NMAA now requires all students interested in participating in Athletics, Marching Band, ROTC, and Bowling to have a sports physical on file with their school district dated AFTER APRIL 1, 2025 and PRIOR TO PARTICIPATION in the sport or activity.


Completed NMAA Pre-Participation Evaluation Form:
Immunization Records (if available)
Eyeglasses or contacts (if needed for vision screening)
Paperwork required by the provider (see below)
Insurance & payment

LCPS is proud to partner with La Clinica De Familia (LCDF) to provide convenient opportunities for students to complete their sports physicals. Attached you will find a flyer and the required LCDF registration packet. Please note:
The packet must be fully completed and signed by a parent/guardian.
Once completed, students should return their forms to their school’s Athletic Coordinator.
LCDF will schedule students for physicals based on the designated day for each school.
Reminder calls will be made the day before each scheduled clinic.

Fri, May 23 | 3:00 PM – 7:30 PM | LCPS Central Office
Students will be pulled out of class briefly to complete their physicals. Parents do not need to be present, though they are welcome to attend if they prefer.
What to Bring:
Completed and signed LCDF CLINIC REGISTRATION PACKET ENGLISH or SPANISH
Completed NMAA PRE-PARTICIPATION EVALUATION (PPE) FORM ENGLISH or SPANISH
Immunization Records (if available)
Eyeglasses or contacts (if needed for vision screening)
Insurance & Payment:
LCDF accepts all insurances.
Medicaid plans cover sports physicals.
If not covered or if uninsured, the cost is $20.
No student will be turned away due to inability to pay.
Additional Notes:
The May 8 and May 23 clinics at LCPS Central Office are open to all students, regardless of school.
All other clinics are for students enrolled at the hosting school site.
If your student cannot attend on the scheduled date or prefers an earlier appointment, please contact one of the LCDF Hub Clinics to schedule. Hours of operation for the Centennial, Rio Grande Prep, Lynn, and Picacho school-based clinics are also attached.
